Pre-Arrival Information Reservations: A deposit of $50.00 is required to reserve a Personal Health Evaluation session. The deposit is non-refundable, but with 48 hours notice may be transferred once to another session. Office hours: The Personal Health Evaluation is being offered in 2 sessions - Tuesday mornings from 09:00 to 13:00 and Wednesday evenings from 18:00 to 21:00. Please feel free to call and leave a message and one of the Robarts Health Program staff will return your call. What to expect: The Personal Health Evaluation is an intensive 2-hour complete health assessment session followed 1 to 2 weeks later by a 1 1/2-hour review session. The Personal Health Evaluation includes: > History and Physical Examination > Blood Testing > EKG > Computerized Diet and Nutritional Evaluation > Computerized Lifestyle Analysis > Complete Physical Health & Fitness Evaluation including computerized body fat analysis, VO2 max. testing, lung function studies (FEV1.0 /FVC), etc. > 1 hour review of results and individualized health prescriptions Additional Medical Testing: There may be additional medical tests recommended on an individual basis as appropriate. These tests are not included in the Personal Health Evaluation Fees, but most are covered by OHIP at this time. > Cardiovascular - An Exercise Stress Test may be recommended for those individuals with a history of heart problems or who are experiencing symptoms which may be cardiac related. > Pulmonary - Pulmonary Function Studies may be recommended for those individuals with a history of chronic lung problems or those who are exhibiting respiratory symptoms. > Additional testing - If the need for any additional laboratory or clinical testing is identified by yourself or Dr. Robarts, they can be arranged to be done at the time of your Personal Health Evaluation. Please let us know of any special tests you may require prior to arrival. You will be contacted by program staff to further assess your needs and assist in schedule preparation. If you have already had these tests within the last year, please bring a copy with you to your Personal Health Evaluation session. Information related to medical problems: The Robarts Health Program is an outpatient program, and thus participants must be able to function independently without the need for nursing care or other assistance. If you are taking medication, we request that you bring them with you to your Personal Health Evaluation session. What to wear: Clothing should permit freedom of movement and include walking or running shoes. Please ensure that the soles of the shoes are clean and free from grit and debris. Women should wear a sports bra or loose-fitting blouse with short sleeves that buttons down the front, and should avoid restrictive undergarments. What to bring: > Your OHIP card. > Your completed Health Screening Questionnaire, Eating Diary, and Lifestyle Questionnaire. > Any medication you may need to take at the time of your appointment (see above for medications). > Warm-up suit, towel, change of clothing (if you don't want to wear your sweaty clothes home). What to do before your appointment: > Abstain from eating or drinking anything but water within 12 hours of the assessment. > Avoid moderate or vigorous physical activity within 12 hours of the assessment. > You will be required to give a urine sample, so please ensure you do not void shortly before your appointment. > Abstain from alcohol consumption within 48 hours of the assessment. Please contact us before taking your diuretic pills if you are taking any that have been prescribed by a physician so that we can advise you when and if you can take your medication. Companions: Spouses or companions are encouraged. The support provided can greatly enhance your response to the program. For the Personal Health Evaluation you may bring a spouse or companion with you for support. Indeed, if they are able to have their own Personal Health Evaluation and attend The 1-Day Robarts Program, you will find that you are greater able to make the necessary life changes in your own life. Medical Records: Recent medical records (within the last year) will be helpful in Dr. Robarts' initial evaluation. Although not required, copies of recent diagnostic test results (EKG's, lab work, treadmill tests, angiograms, etc.) and/ or discharge summaries from any recent hospitalizations will be helpful. Please bring them with you at the time of registration. Note: The copies provided to us will become part of your Robarts Health Program Medical Chart which will remain here. If you wish to make copies for yourself you are free to do so, or to request them directly from your physician/ lab, etc. Location: The Personal Health Evaluations are done in our offices at Suite 303, 2338 Hurontario Street. This building is the 3rd building north of The Queensway on the west side of Hurontario Street. Purpose: Although the Personal Health Evaluation is a useful tool in itself, the Personal Health Evaluation has been designed as an initial intake assessment and review that will facilitate your being able to maximize the benefit of The 1-Day Robarts Program.
